Join stakeholders from around the Chicago region to get introduced to how Chicago and Cook County’s Homeless Continua of Care are creating a new re-housing approach to change the course of homelessness through collaboration, data-informed population needs, and equitable access to housing and resources.
The program will include:
- An overview of the process for coordinating entry into housing for households facing homelessness in Chicago and Cook County, including progress to date and a discussion of lessons learned.
- Lived Experience Testimonials
- A moderated panel discussion on how Coordinated Entry Systems are transforming how our communities address ending homelessness through increased collaboration and strengthened data collection that accounts for the diversity of needs, successfully incorporates partner systems, and creates an accessible, equitable path to housing.
The program starts at 8:30 am, with registration and continental breakfast beginning at 8:00 am.
